HUD Continuums of Care

A Continuum of Care (CoC) is the regional body HUD funds to coordinate homelessness services. HUD reports its annual Point-in-Time homeless count by CoC — not by county — so the CoC is the unit that matters for homelessness data.

Why this page exists
CoC boundaries do not line up with counties: some cover a city and a county together, many “Balance of State” CoCs span dozens of rural counties, and in New England they are built from towns. A few counties contain more than one CoC. Because HUD publishes no county crosswalk, working with homelessness data means working directly with the CoCs — so this is a plain directory of all of them. Contact details are being compiled directly from the CoCs, and there are gaps. Where a website, email, or phone is missing it simply has not been confirmed yet — it will be filled in as responses come back. CoC names and numbers are complete.
